Following is a list of a few simple rules we will need for everyone to follow: 1. You are responsible for your own setup including items such as tables, chairs,money box or any other items you feel you may need. 2. If you need electric you must indicate so on the form below and you must bring heavy duty cords and duct tape. 3. Setup will begin at 8:00 a.m. that morning and we will notify you where you are to setup by Friday, October 1, 2010. If you have not heard by Friday you may call Sharon at 630-4665 or 482-2866. 4. Parent supervision is required for younger children, for setup and for handling of money and change, if they are unable to do so on their own. 5. Parent or guardian must sign waiver and release form in order for child to participate. 6. Parents must move their vehicles immediately after unloading, so by 9:00 customers for the event will have parking space available. Some parking will be available at the Old National Bank and German American Bank parking lots. In case of weather related problems we ask that you use your own judgment unless we announce the cancellation of the event on WITZ. TO ENTER YOUR BOOTH: PLEASE COMPLETE THE ATTACHED REGISTRATION/ WAIVER AND RELEASE FORM AND MAIL IT TO SHARON WILZ, 572 43RD ST., JASPER, IN 47546 BY FRIDAY, SEPTEMBER 24. FOR ADDITIONAL INFORMATION OR FOR QUESTIONS PLEASE CALL 630-4665. IF MORE THAN ONE WILL BE DOING THE BOOTH TOGETHER PLEASE ONLY SEND IN ONE FORM AND LIST THE NAMES OF ALL ON THE FORM AND HAVE ALL PARENTS SIGN THE WAIVER FORM. IF YOU WISH TO BE BY ANOTHER BOOTH PLEASE MAIL THE FORMS TOGETHER REQUESTING THAT YOU BE PLACED BY EACH OTHER.
